The first cases ascribed to this pathogen were described in 1946 and caused ulcerative skin lesions among American soldiers in the Pacific Islands. Today, infections by A. haemolyticum are rare but most commonly cause pharyngitis in adolescents and young adults or skin and soft-tissue infections in immunocompromised populations.
An Arcanobacterium haemolyticum infection is any of several types of infection with the gram-positive bacillus Arcanobacterium haemolyticum. It can cause an acute pharyngitis, and it may cause an exanthem characterized by an erythematous, morbilliform or scarlatiniform eruption involving the trunk and extremities. Objective: To describe a case of severe sepsis, cavitary pneumonia, and pyomyositis caused by Arcanobacterium haemolyticum. Case summary: An 18-year-old male with a medical history significant for mild asthma presented to the emergency department complaining of a 7-day history of fever, diffuse myalgias, nausea, vomiting, diarrhea, and pain in his right upper quadrant, right shoulder, and left thigh. Arcanobacterium haemolyticum was first described by MacLean et al.
Fever, pharyngeal exudate, lymphadenopathy, rash, and pruritus are common, but palatal petechiae and strawberry tongue are absent. In an 18-month period all throat cultures in our laboratory were examined for Arcanobacterium haemolyticum and clinical information was obtained. The great majority of the patients suffered from pharyngitis or tonsillitis, accompanied by a rash in 46% of cases. One third of the patients had a history of recurrent tonsillitis. Arcanobacterium haemolyticum is a catalase negative gram-positive rod, showing narrow zones of beta-haemolysis when cultured on human or rabbit blood agar6, 10. The organism, Arcanobacterium haemolyticum is rarely isolated from clinical),material either in human or veterinary medicine. As for our country, there is only one official report with regard to the isolation Arcanobacterium haemolyticum osteomyelitis and sepsis: a diagnostic conundrum.
